Our Action Plan

Collective action requires people to be committed to the process, vision, and goals of the work. An Action Plan translates objectives into specific activities that organizations involved in the MAPP process will carry out. Once the 2024 CHIP report is finalized, this page will have an Organizational Objective Submission form for representatives to submit to the All in For Health partnership to track progress toward the CHIP priority areas and goals. We will also share and update the Action Plan on this page for organizations and community members to review.
