All in for Health Partnership
All in for Health is a community-wide initiative involving the collaborative engagement and expertise of organizations and individuals from multiple sectors across Jackson and Josephine Counties. All in for Health invites individuals and organizations throughout the region to come together to create a healthier community because “A healthy community is everyone’s business."
As a community, we recognize the circumstances in which people are born, live, learn, work and age directly shape their health and well-being, and that no single organization or sector can improve the health of the community alone. All in for Health’s platform is intended to help everyone learn about the health and wellbeing of Jackson and Josephine Counties. It provides local data, resources, best practices, grant opportunities and community events, providing information on a broad range of topics affecting the health of our community while highlighting local assets.
All in for Health’s hope is that this site and data will also support our efforts to continue to do a community wide, collaborative Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) process. We look forward to what we can accomplish together in our shared work towards building community health and wellbeing for all people in the region.
Community Health Assessment
The 2023 Community Health Assessment of Jackson and Josephine Counties aims to meet the requirements of partners as well as develop a shared understanding of community health in order to guide collaborative community health improvement efforts.
Community Health Survey
From April through July 2023, a community survey was developed and distributed in both paper and electronic formats across Jackson and Josephine Counties. The survey focused on community members’ and providers’ perceptions of the community, top health concerns, and barriers to accessing health and social services.
Community Health Improvement Plan
The 2019-2022 All in for Health: Jackson and Josephine Counties Community Health Improvement Plan (CHIP) is a community-level strategic plan that outlines how multiple organizational partners, working together, will collectively address priority health issues over the next three years to improve overall health and well-being within the region.
